NCVIN update: diabetes foot care profiles, heart attack and stroke prevention drive, CVD intelligence packs and other network news

NCVIN update

National Cardiovascular Intelligence Network (NCVIN)

Issue 3, September 2017

Dear colleague

We've recently completed updates for some of our most well-established and used outputs: CVD primary care intelligence packs and Diabetes foot care profiles. We hope that you find the new data in these resources useful and would be keen to hear from you about how you might have put what you've found out from looking at them into practice on the ground. Do let us know how you've used these and if you have any thoughts for how they might be improved by emailing ncvin@phe.gov.uk

Like many of you, we're also beginning to look ahead to what our work programme might include in 2018/19. It still seems some time off but we know that we'll be trying to make detailed plans before long. If you have any suggestions for how our work programme can best meet your needs next year, please let us know by emailing ncvin@phe.gov.uk

Diabetes foot care profiles update

Updated versions of the CCG level diabetes foot care profiles were published on the 5 September. The profiles provide data and analysis on inpatient spells and procedures relating to diabetic foot disease. The most recent profiles have undergone significant methodological improvements since previous iterations - most notably clinical commissioning group (CCG) rates of diabetic amputations have now been standardised for non-modifiable risk factors such as ethnicity and age. Alongside the profiles, there is underlying data, a technical document and England level summary.

An accompanying paper in The Lancet was also published on the 11 September, co-authored by members of NCVIN. The paper highlights the significant global burden of diabetic lower-limb amputations and suggests strategies for tackling the variation seen in amputation rates.

CVD primary care intelligence packs updated

The CVD primary care intelligence packs can help healthcare professionals make decisions about how to improve local primary care. There is a pack with local cardiovascular disease (CVD) information for each CCG. The packs look at prevention, diagnosis, care and outcomes and allow for comparison between CCGs and between GP practices. Use the pack to identify local variation and ask questions about where and how the quality of care may be improved.

New drive to prevent heart attacks and strokes

The NHS, working with Public Health England (PHE) has announced a new drive to prevent heart attacks and strokes saving thousands of lives by taking a more integrated approach to cardiovascular care. New PHE analysis suggests that there is now an opportunity to prevent more than 9,000 heart attacks and at least 14,000 strokes over the next three years with better detection and management of high blood pressure, high cholesterol and atrial fibrillation. PHE and NHS England have written to all 44 sustainability and transformation partnerships (STPs), drawing attention to this prevention opportunity, and sharing with them the heart attack and stroke prevention data for local STP areas.

News from our partners

National Diabetes Audit (NDA)

A new National Diabetes Footcare Audit report will be published on 12 October which looks at hospital admissions for foot-related complications. Also published on 12 October is the National Pregnancy in Diabetes Audit 2016.

A new NDA initiative to support specialist diabetes services to implement improvement is in development. Over the next three years, the NDA team will be supporting quality improvement projects in four specialist areas: inpatient care, pregnancy care, footcare and transition. If you'd like more information about this work or would like to be involved, please email the NDA.

NICOR

NICOR has transitioned from University College London and is now being hosted by Barts Health NHS Trust.

The NICOR 2016 National Heart Failure Audit report was published on 10 August 2017. The report highlights improved access to evidence-based care and lower in-hospital mortality rates. While the audit reports several improvements, it also found that the quality of care varied from one hospital to another, and within a hospital, between the specialist and other wards.

Over the next few years NICOR will be focusing on supporting quality improvement within acute cardiac care. NICOR is in the process of developing quality improvement measures that can be used by a variety of stakeholder including clinicians and hospital staff, commissioners and patients to support local quality improvement. If you would like to be involved in this process please email NICOR.

Sentinel Stroke National Audit Programme (SSNAP)

Annual data for stroke patients admitted and discharged in April 2016 – March 2017 is now available to the public including team-level thrombectomy data for the first time. Data analysis is underway for the April – July 2016 reporting period and results will be made public in November 2017.

SSNAP will be at stand B2 at the 2017 UK Stroke Forum. Visit the stand for copies of SSNAP's latest annual report, patient-friendly reports and profession-specific concise guides for stroke.

Think Kidneys

Acute kidney injury (AKI)

Think Kidneys have launched a practical guide to help with recognising and responding to AKI for community staff, designed for those working in care homes, community nurses or ambulance crews. You can also email Think Kidneys to request hard copies.

They have now launched resources for educators of undergraduate nurses. These include guidance, lesson plans, slide sets and a video scenario for student nurses in years one to three.

Think Kidneys will begin work on CCG level data for AKI in the autumn.

Chronic kidney disease (CKD)

The Transforming Participation in CKD programme aimed to develop ways to support people self-manage their kidney disease. With the UK Renal Registry coordinating data collection, more than 4,000 symptom scores have been collected from over 3,700 kidney patients using paper questionnaires across 14 renal units. Examples of how units collected and used the data are available. The programme has also launched an Intervention Toolkit.

The patient reported experience measures (PREM) for 2017 has now been collected at adult renal units across England and Wales. The UK Renal Registry in partnership with the University of Hertfordshire will analyse the results before publishing them later this year. The report for the 2016 pilot PREM is available.

Kidney Quality Improvement Partnership (KQuIP)

The KQuIP Hub is a growing online repository of learning and quality improvement resources. KQuIP is working to deliver the spread and sustainability of embedded quality improvement through established regional days. There are also three national priority project areas for Transplant First ; MAGIC (Managing Access by Generating Improvements in Cannulation); and Home Therapies.

You can see KQuIP's quarterly E-Newsletter from July or sign up for regular programme updates on the KQuIP website.
