BREAKING: Senior military officials sanctioned for more than 500 cases of serious misconduct

But despite these cases, USA TODAY has found, the Pentagon has no uniform way to track misconduct, and the office created to fix the problem closed before it could finish the job, much to the dismay of its creator, former Defense secretary Chuck Hagel.
